Submit an Awards Program Application

1. SIGN UP
If you submitted an Award application last year, please skip this step and log in. To submit an awards application, you must first sign up for an awards account by clicking on Sign Up. Your user name will be your email address, and you will choose your own Awards password. Please note that this login is different from the personal login you use for the main Practice Greenhealth website. You want to be able to give this Awards login to anyone at your organization that needs to input data or review the application, so choose an easy-to-remember password.

2. LOG IN
Once you have created an Awards account, simply log in using your registered email address and chosen password. If you have forgotten your password, please use the "forgot password" function. A link will be emailed to you immediately.

3. GO TO MY AWARDS HOME
Once you are logged in, click on "My Awards Home" in the upper right side of the screen to begin or resume an application. This page will become your friend during the entire Awards application process.

Remember your Award plaque will bear the facility or business name as you enter it on the first page of the Award application. If you are a health care facility, please check with your Health System administration, to see if you need to use a standardized facility name. Please take care to enter your email address properly - we use this address for winner notification and a number of important emails. And do enter your health system, if appropriate, as Awards will be handed out by health system in Denver at CleanMed 2012.

4. SHARING YOUR LOGIN
You may share your Awards login (your email and chosen password from Step #1 above) with anyone in your facility that needs to enter data or review the application.  Each facility or business will have only one login for the Awards account, but many staff can enter or review data. Encourage your Facilities, Food Services, EVS, Materials Management, etc. staff to enter data straight into the application to save you time and possible errors.

A brief overview of the Award Types:
Please note that some of the minimum requirements have increased and the recycling rates include only those items in the revised Appendix A (diversion is excluded).

If you are a health care facility you may apply for the following Awards:

The Partner Recognition Award recognizes healthcare facilities that have begun work on environmental improvements, have achieved progress in several areas, and have at least a 10% recycling rate for their total waste stream. This is a good introductory Award to our program. For more information see: http://practicegreenhealth.org/awards/award-types#pr

The Partner for Change Award is a non-competitive Award (if you meet the criteria, you win the Award) that recognizes health care facilities that have established environmental programs and are working towards sustainability, and can report success in a number of areas. Facilities must have at least a 15% recycling rate, have significantly reduced or eliminated mercury, and have made progress towards becoming sustainable. For more information see: http://practicegreenhealth.org/awards/award-types#pfc

The Partner for Change, with Distinction Award is a competitive Award that recognizes healthcare facilities that have well established environmental programs and are the cream of the PFC crop. These facilities must have a recycling rate of 20% or higher, and an RMW generation rate of 10% or lower. Distinction winners must be able to demonstrate progress in the basics, such as an EPP policy, RMW reduction techniques, Green Cleaning, and IPM, but also show progress in Food, Energy and Water. Distinction winners have a “culture of sustainability” and are leaders in their communities and in the healthcare sector. For more information see: http://practicegreenhealth.org/awards/award-types#pfc

The Environmental Leadership Award is Practice Greenhealth’s premier Award recognizing health care facilities that exemplify environmental excellence and are setting the highest standards for environmental practices and sustainability in health care. Progress in all areas and real innovation is required to win this Award. This is our most competitive Award and only a few new members are inducted each year. Source reduction is valued more highly than recycling alone, and the recycling rate must be 25% or greater with an RMW generation rate of 8% or less. See more information at: http://practicegreenhealth.org/awards/award-types#pfc

The DEHP-Free Award is given to the hospital or health system that demonstrates the most success with eliminating DEHP- and PVC-containing medical devices. This Award is generously made possible by Hospira, maker of DEHP-free VisIV® Containers. , The DEHP-Free Award application has been integrated into the Partner for Change Award application as Appendix C. For more information see: http://practicegreenhealth.org/awards/award-types#dehp

The Making Medicine Mercury-Free Award recognizes facilities that have virtually eliminated mercury from their facilities and have made a commitment to continue to be "mercury free." Your facility must have all of their mercury- free policies in place and be able to indicate mercury reduction or elimination in all areas of the application (with the exception of construction) to win this Award. For more information see: http://practicegreenhealth.org/awards/award-types#mmmf

If you are a business member, or a strategic resource network member, you may apply for the Champion for Change Award.

The Champion for Change Award recognizes businesses and organizations that demonstrate successful accomplishments in ‘greening’ their own organization and have assisted their associated healthcare facility clients/members/customers in improving their environmental performance. For more information see: http://practicegreenhealth.org/awards/award-types#cfc

If you are a Healthcare System, and have joined Practice Greenhealth as a health system, you may apply for the Systems for Change Award.

The System for Change Award recognizes health systems that are working cohesively to set system goals, track data, benchmark, and share successes in sustainability. A minimum of 20% of the health system’s facilities must also apply for and meet or exceed the minimum criteria for the Partner for Change Award the same year in order for the system to receive this Award.
